Update Application Form and Information

This form can be used only by students who met both of the following criteria. Students must: 

  1. have been enrolled through The Graduate College during the last year
  2. wish to enroll in the same program of study

Example: if you were enrolled in program A for the fall term but will not enroll until the next fall term in program A.

If it has been more than a year since your last enrollment, you must complete an ApplyTexas admissions application and pay the fee associated with the application.

If you wish to enroll in a different program or status, you must complete an ApplyTexas admissions application and pay the fee associated with the application.

Universities in Texas are required by state law to collect proof of bacterial meningitis vaccination or proof of an exemption for incoming students under the age of twenty-two. Texas State University is working with Magnus Health Student Medical records (SMR) to collect, review, and confirm our students' bacterial meningitis vaccination information. Please fill out the Update Application Form below.  If you are under the age of twenty-two, you must submit documentation through Magnus Health SMR before you can register.

Please Note: If you have a new address or phone number, please update your information in the Texas State University student information system through Texas State Self-Service.
