For Changes or Corrections to Name, Social Security Number, or Date of Birth:

To protect the integrity of your Academic Record, you will need to provide written authorization and documentation verifying your name, social security number, or date of birth.  Please complete the Personal Information Update Form (PDF) and submit it to the Graduate College.  Two of the following documents must be presented in person at the Office of the Graduate College, J.C. Kellam Suite 280:

• Driver’s License
• Divorce/Marriage Certificate
• Court Order for Legal Name Change
• Social Security Card (Required for Student Employees)
• Birth Certificate
• Passport

Although faxes cannot be accepted, you may choose to mail the required documents to the Office of the Graduate College.   A Personal Information Update Form (PDF) must be submitted along with notarized photocopies of two of the required documents listed above.  Return by mail to:

**NOTE: Any student that is also a current employee at the university must submit a copy of their Social Security Card with full name indicated.  This is a requirement of Human Resources and no exceptions are allowed.
Any change in name will be reflected on the following:

• Class Rosters
• Official Transcripts
• Student Information Profile
• Diploma

Graduating Candidates
The name on your diploma will appear as the name we have on file (class schedule/transcripts).  To change your name to your legal name you must follow the procedure stated above.

Please note:  The deadline for changing Fall 2009 graduates' legal names is January 7, 2010.

The name on the Commencement Program and diploma may be different for changes made after November 9, 2009.
